Personalised Care to Support Independent Living
At Wongaburra Home Care, we provide personalised Support at Home (SAH) services to help older Australians live safely, comfortably and independently in their own homes for as long as possible.
The Support at Home Program, introduced under the Aged Care Act 2024, replaces the previous Home Care Packages (HCP) and Commonwealth Home Support Programme (CHSP).
It simplifies the system by combining them into one program focused on your goals, wellbeing, and independence, while giving you greater flexibility and control over the services you receive.
We’ll work closely with you to design a tailored Support Plan that reflects your needs, preferences and lifestyle.

What is Support at Home?
Support at Home is the new Australian Government program that replaces Home Care Packages and CHSP. It provides one streamlined system to fund services that help older people remain independent and safe at home.
Who is eligible?
Eligibility is determined through an assessment by My Aged Care, based on your support needs and ability to live independently at home.
What services can I receive?
Support at Home covers personal care, domestic assistance, meals, transport, social activities, nursing, allied health, and access to aids or home modifications.
What will it cost me?
Support at Home is subsidised by the Australian Government. You may be asked to contribute towards the cost of your services depending on your income and circumstances. Wongaburra will always explain any contribution clearly before you start.
Already receiving a Home Care Package?
If you were receiving a Home Care Package before 1 November 2025, your current funding and services continue under some of your existing arrangements. You are considered a grandfathered client and there is no change to your care or out of pocket fees.
